I'm using Ooma. You can get the boxes in a lot of places (got mine at Costco). It's cheap and so far works pretty well. The upfront cost is more than most but you recover it pretty quickly with the low monthly fee.
My only tiny beef is that if you talk to someone that has a different voip setup you can tell there is just a tiny bit of lag. It doesn't seem to happen ooma to ooma but, say, ooma to vonage.
You can do Skype and Google for free and Skype actually has some LD plans.
