If you don't count RGS and the other offshores my guess would be:
Total handle: Xpressbet, AmWager, TVG, TwinSpires.
Number of customers: TwinSpires, TVG, Xpressbet.
But that's just a guess.

ADW INFORMATION
A lot of ADW providers operate out of Oregon. If you go to their site, at the Oregon Racing Commission, you can figure some of this out. There are also entities that work with multiple ADWs and provide rebates based upon volume. RGS is Racing and Gaming Services and they have very very high per capita wagering individuals, and they are compliant with the IRS etc.

US racing did about 9.8 Billion in off track handle in 2017. All the sources listed on the Oregon site did 3.84 Billion(which I assume includes Canada betting as well-but I am not even taking that into account). That is a heck of a lot of money being bet elsewhere (whale Money I assume). About 60% of off track money is not coming in from the major adw's?
Am I missing something?

Money bet on other tracks by people who are actually at a track or OTB, like a guy (for example) sitting in any racetrack but Keeneland and betting on Keeneland? Not sure if that would be included.

I would venture a wild guess that the whales don't bet through major ADWs, so you did answer your own question.

Interesting statistics came out of the forum. Roughly $2 billion a year, or just less than 20% of total pari-mutuel handle, is said to come from high-volume shops. One of them—Elite Turf Club, which has only 11 customers—accounts for 10% of handle nationwide each year.

As I recall, RGS and Elite Turf Club do not use Oregon as their wagering hub, but LVDC (Las Vegas Dissemination Company) instead. It would appear that Elite clients average around $100M each.
