What are the rules, if any, that require a program to list the assistant trainer alongside the trainer of record? I imagine it might vary by jurisdiction, and/or that it might be at the discretion of the trainer?

I've not ever seen this until recently, in New York on Belmont Day. In the program book, which normally I wouldn't have, but on big days purchase. (Having figs and Bris pps.)
I was pleased, I thought it was a fine and thoughtful thing, sharing credit with one's assistant, particularly when that assistant handles all of a trainer's string as Sheets does for Steve A's Aqueduct and Belmont runners in New York throughout the year. This may not be the reason behind adding one's assistant, I don't know. But it was nice to see.

In the 5th for Hoppsey, trainer line read:
Steve Asmussen (Toby Sheets)
