The USTA's OLD Track page (http://web.archive.org/web/20040409024639/www.ustrotting.com/trackside/trackfacts/bmlp.htm) is a good resource for this and more info (even though incomplete).
The new track page is revised and contains lesser information.

I missed one more track:

Hanover Raceway in Ontario.


With the Ontario sired races going on a lot of horses are coming from these small tracks to Mohawk and I thought that this info was pertinent. Most of these tracks are 1/2 milers and its one thing to score from the outside post at these 1/2 mile tracks than coming from a trailing position.
