Does anyone have a copy of the old F.I.G.S Form

Does anyone have a copy of the old F.I.G.S form, I know it went out of business about 20 years ago, but does anyone have a copy ? Also my pick for the Kentucky Derby is Big Brown, this horse looks strong, even though he has just 3 races, he has smashed his fields.
