Jockey Frank Hayes suffered a fatal heart attack in the middle of a race at New York's Belmont Park in 1923. Legend has it that his horse, Sweet Kiss, went on to cross the finish line first. If true, that would make Hayes the only dead jockey ever to win a horse race.

Same thing happened at the standard breds: driver had an MI, died, finished in front.

There is another example from Bay Meadows when Ralph Neves was reported dead in the newspaper (he had left the track for the hospital unconscious and reported dead) then woke up, and rode several horse in the money later in the program.
