Not knowing exactly what kind of system or systems were used to contain the data, I can not be too specific, however, knowing enough about operating systems and trusted computer systems then reading the article, my knee-jerk reaction is to believe there was a massive failure on the part of key individuals to do their job. If it was up to me, I'd terminate the lot of them. In this day and age, there is no excuse for not having systems locked down and having the audit trails completely secured -- none.
And the more that comes out about this situation the less and less secure we should all feel about our security agencies.
